Cooking is universal; exposure to smoke should not be. Yet household energy remains one of the clearest ways in which poverty, infrastructure and gendered daily routines can become biological exposure. The choice of cooking fuel is rarely just an individual health preference. It reflects price, reliability, housing, electricity access, cultural practice and the technologies a household can sustain.
Multi-country PURE research found substantial differences in health risk and particulate exposure across household-fuel categories. PURE-AIR then made the equity argument more precise by directly measuring kitchen and personal PM2.5 in rural communities across eight countries. Black-carbon measurements showed a similar gradient and also demonstrated that ventilation and housing modify what people breathe. Later work on fuel switching showed that transitions are shaped not only by households, but by communities and countries.
The result is a more realistic clean-energy story. Adopting a cleaner fuel does not guarantee complete exposure removal if households continue ‘fuel stacking’, background pollution remains high, or the new fuel is unreliable or unaffordable. Energy policy becomes health policy because sustained exposure reduction depends on infrastructure and household economics as much as on individual knowledge.
