Community engagement is sometimes treated as a communication stage: researchers design an intervention and then explain it to the people expected to use it. That misses a more important role. Communities often understand barriers, language, relationships and feasibility that are invisible from the centre of a research institution. Participation can therefore change the intervention itself.
Implementation-readiness work has treated communities, organizations and researchers as partners in preparing for complex interventions. Remote hypertension studies invited community input into culturally appropriate messages and examined feedback from participants and leadership. Participatory cervical-screening projects work with communities to define barriers and acceptable strategies, while adolescent stigma research adapted language and activities with young women. Multi-country adolescent NCD programmes extend the idea toward co-design.
Participation has levels. Consultation asks for opinions. Co-design changes components. Shared governance changes who has authority. Community-led research can change the question itself. The public narrative should be specific about which level occurred rather than using ‘community engagement’ as a flattering but vague label.
